1. Request by email sent to info@artprize.co.uk and allowing 14 days for us to acknowledge and reply with instructions
2. Payment of a fee of £12.50 (via bank transfer or card transaction via PayPal)
3. Two appropriate documents proving your identity, one of which is an official identification document with photo id (e.g. copies of passport or driving licence), and also one which shows evidence of address (e.g. bank statement or utility bill).
We may withhold such personal information to the extent permitted by law.
